Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24 hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment and we appreciate your cooperation. There will be a $50 charge per hour short notice cancellation fee for all appointments cancelled within 24 hours of appointment time.
Insurance
Our office is committed to helping you maximize your dental benefits. Because dental insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of dental insurance contracts. Insurance coverage is subject to limitations, exclusions, waiting periods, frequency, age restrictions, deductibles and maximums which are your responsibility. Your estimated patient portion must be paid at the time service is rendered. As a service to our patients, we bill dental insurance companies for services on your behalf.
In order for dental insurance claims to be processed in an efficient and timely manner our office and the insurance companies require the following information:
1) The correct spelling of your name the insurance company has for you on file.
2) Your ID# and Group#.
3) Your birthdate and Social Security Number
4)Your correct mailing address they have on file for you.
Insurance companies cross reference all of the above when we call them to verify your benefits and also to process claims. All claims are filed electronically. We rarely have issues with claims being processed and most claims process within 3 to 30 days depending on the specific insurance company. When we do have an insurance issue, you may need to contact your insurance company to have your claim processed. Dental insurance is a contract between you and them.
If you feel uncomfortable providing the requested information we can supply you with a completed claim form that you are able to bill on your own. In this instance, full payment will be due for services rendered and your insurance company would than reimburse you for the covered benefit amount.
